Authorization for Release of Confidential or Protected Health Information
Individuals
If you are an individual and would like to request an EMS Patient Care Report, please call 772-226-3952 to set a time to pick up the report at the EOC, 4225 43rd Avenue. Please note the individual MUST BE the patient, or the individual with the legal authorization to grant the release of the patient’s protected health information (e.g. parent of a minor or legal representative of the estate of a decedent or PHI Authorization for Release.)
Attorneys
If you are an attorney’s office and need to request an EMS run report on your client’s behalf, be sure to include authorization of release and/or subpoena without deposition. Requests cannot be processed without the proper release authority. Submit your request to FireEMSrecordrequest@ircgov.com If you need an ambulance bill regarding your client’s service please use Chartswap: https://www.chartswap.com/ Indian River County ALS.
Insurance Companies
If you are an insurance company requesting an incident report related to a house or car fire, etc., please submit your request to FireEMSrecordrequest@ircgov.com and include the date of incident and location.
Subpeonas
Indian River County Fire Rescue Record’s Department accepts subpoenas via process server or certified mail. Subpoenas for Deposition not received through a process server or certified mail may be returned to sender. Subpoenas sent via certified mail or process server are accepted at:

Indian River County Fire Rescue can provide individuals with:
- Fire reports
- EMS/Medical transport reports
- IRCFR standard public records and reports
Information will not be released to outside parties, unless ALL criteria has been met.
